We are honored to have the opportunity to partner with you in the education of your child. Following is a list of the items that you may need for the enrollment process.
Official withdrawal form from previous school attended
Current immunization record copies which, by law, MUST be provided prior to entering the classroom
Copy of birth certificate
Proof of Residency (i.e. copy of water, gas, electric bill, lease agreement, etc.)
Transfer/withdrawal grades from previous school when entering mid-year
Notarized guardianship/temporary custody document if applicable
Court documents - if there is sole/restricted parental contact by court order
Open enrollment form - if you live out of our district
